Once you submit this form, a team member will contact you as soon as possible. Please allow up to 72 hours for processing. All requests are subject to approval, and you will be notified once a decision has been made.
Please note: As of 4/1/25, all requests must be submitted through this form. Requests made via email, phone, or text will not be accepted.

FREEZING RECURRING MEMBERSHIPS
We offer a variety of membership options to suit different pricing and commitment preferences. Please refer to the details below for information on cancellation policies and required notice periods.
Memberships may be frozen up to two (2) times per calendar year, for a total of no more than 60 days.
Freeze requests exceeding 60 days must be accompanied by a doctor’s note clearly stating that you are unable to participate in classes at CORE34 Pilates during the specified time period.
CANCELING RECURRING MEMBERSHIPS

Cancellations require a 30-day notice from the autopay date.
Memberships may only be canceled prior to the contract expiration if accompanied by a doctor’s note or proof of relocation beyond a 20-mile radius from your home studio.
If you choose to cancel your membership before your contract ends, you are responsible for completing the terms of your signed agreement and paying for the current month in full.
Partial-month refunds will not be issued.

TERMS AND CONDITIONS
All class sales and memberships are final.
Classes are non-transferable and not eligible for refunds or exchanges.
If a credit card expires or a scheduled membership payment is declined, the member remains responsible for fulfilling the contractual payment.
